The amount listed in the work order is an estimate of the labor requested only, and does not include any parts or additional labor that may be required once service has begun.
THIS AGREEMENT is a release of the CUSTOMER to sue for injuries or death resulting from use of the equipment after service/repair. The CUSTOMER expressly assumes all risks of scuba diving, skin diving, or any other activity related in any way to the use of this equipment.
The CUSTOMER also understands that Splash Water Sports, Inc. and its employees, owners, officers or agents, shall not be held liable or responsible in any way for any injury, death, or other damages to CUSTOMER or his/her family, heirs, or assigns which may occur as a result of use of the equipment after service/repair.
